Risk factors for Neisseria gonorrhoeae and genital Chlamydia trachomatis infections among male outpatients attending sexually transmitted disease clinic in Zhuhai city, 2015-2018

Abstract:Objective To investigate the prevalence and risk factors of Neisseria gonorrhoeae (NG) and genital Chlamydia trachomatis (GCT) infections in male patients attending sexually transmitted disease (STD) clinic in Zhuhai city, and to provide a scientific basis for conducting intervention and prevention work in this population. Methods Based on HIV sentinel surveillance, male patients attending STD clinic of Zhuhai Center for Chronic Disease Prevention & Treatment from April to June each year during 2015-2018 served as the research subjects, and then questionnaire survey and detection of blood and urine samples were conducted. Results A total of 764 outpatients were enrolled in this study, and their average ages were (36.73±12.02) years. The married patients accounted for 62.96% (481/764), and the Cantonese patients 88.74% (678/764). The positive rates of NG, GCT, HIV and syphilis infection were 1.83% (14/764), 6.15% (47/764), 2.23% (17/764) and 11.65% (89/764), respectively. Logistic regression analysis showed that having ever injected drugs was the risk factor for NG infection (OR=27.19, 95%CI=2.32-319.03, P<0.001). Having sex with temporary sexual partners in the past three months (OR=2.44, 95%CI=1.19-4.98) and having anal sex with men (OR=2.76, 95%CI=1.23-6.24) were the risk factors for GCT infection. In addition, urinary leukocyte abnormality was positively correlated with NG and GCT infections. The proportion of urinary leukocyte abnormality was higher in outpatients with NG infection than in ones without NG infection (OR=49.63) as well as higher in outpatients with GCT infection than in ones without GCT infection (OR=12.32). Conclusions The infection rates of NG and GCT were high among male outpatients in the STD clinic in Zhuhai city in 2015-2018. It is necessary to strengthen high-risk behavior intervention among male outpatients in the STD clinic, who are considered as the most accessible population for STD prevention and control, and improve the effectiveness of STD prevention and control.
