Latent tuberculosis infection among freshmen in a college in Changping District, Beijing

Abstract:Objective To investigate the prevalence of latent tuberculosis infection (LTBI) among freshmen so as to take preventive interventions. Methods Four hundred and twenty newly-admitted college students with positive results of tuberculin skin test (TST, diameter ≥ 10mm) were randomly enrolled from a college in Changping District, Beijing in October 2010. Enzyme-linked immunospot assay (ELISPOT) was used to detect the peripheral blood T lymphocytes that secreted IFN-γ after antigen stimulation, spot forming cells (SFCs), in all participants. Then the participants with positive results of ELISPOT were routinely monitored for 3 years to investigate the incidence of active tuberculosis (TB). Results The overall positive rate of LTBI by ELISPOT was 41.2%. No statistically significant difference was found in the positive rates of between the participants with and without BCG vaccination (41.5% vs. 40.0%, χ2=0.064, P=0.447), but there were statistically significant differences in the positive rates among the groups with TST induration of 10-14 mm, 15-19 mm and ≥ 20 mm (37.6%, 45.4% and 64.3% respectively, χ2=8.408, P=0.015). The 173 untreated students with ELISPOT+/TST+ were followed up for 3 years, and none was found to develop active TB. Conclusions The prevalence of latent tuberculosis infection among the freshmen of the college in Changping District, Beijing is high. Only taking ELISPOT+/TST+ as the indicators for tuberculosis prophylaxis treatment is not enough.
