Attribution analysis on the influence of sleep quality on cognitive frailty of community-dwelling elderly people

Abstract:Objective To explore the role of sleep quality and sleep duration in cognitive frailty of the elderly living in the communities. Methods This study is cross-sectional. The Pittsburgh Sleep Quality Index (PSQI) was used to assess the sleep quality of the elderly, the Chinese version of the Ascertain Dementia-8(AD-8)questionnaire was applied to evaluating cognitive status, and the Frail Scale was employed to appraise physical frailty. Logistic regression analysis and multivariate-adjustedpopulation attribution risk analysis were performed to identify the effect of sleep on cognitive frailty. Results In this study, a total of 955 community-dwelling elderly subjects were surveyed, including 425 (44.5%) males and 530 (55.5%) females. The detection rate of cognitive frailty was 8.7%, and the detection rate of sleep disorder 24.5%. Multivariate logistic analysis showed that after adjusting for mixed factors like gender, age, body mass index, marriage, physical exercise, noontime snooze, chronic disease, body pain, depression and anxiety,senior citizenswith sleep disorder (OR=1.891, 95%CI:1.087-3.291) and sleep time < 6 hours (OR=1.859, 95%CI:1.044-3.312) were more likely to develop cognitive frailty. The risk of cognitive frailty increased with the gradual increment of PSQI score. The attributable risk of sleep quality and sleep duration to the cognitive frailty population was 25.53% and 23.95%, respectively. Conclusion The sleep quality of the elderly living in the communities was related to the degree of cognitive frailty. Poor sleep quality and short sleep duration were important factors leading to cognitive frailty in this study population. Taking targeted measures to intervene in sleep quality may be an important approach to alleviate cognitive frailty of the elderly.
