Abstract:Objective To investigate the changing tendency of maternal mortality rate and its influencing factors in Shannan city from 2010 to 2017. Methods We retrospectively analyzed the death report cards and case investigation reports of the maternal deaths in Shannan city during 2010-2017 with the WHO twelve-grade classification standard on maternal deaths. Results The maternal mortality rate in Shannan city during 2010-2017 presented a declining tendency year by year. The mortality rate in 2015 (which decreased from 194.98/100,000 in 2010 to 38.03/100,000) was the lowest, but there was an accidental rise in 2017 (105.22/100,000 in Shannan city, 102/100,000 in the autonomous region and 19.6/100,000 in the whole country). The hospital delivery rates during 2010-2017 increased by 12.15% annually, increasing from 61.37% in 2010 to 99.37% in 2017. There was a negative correlation between the hospital delivery rate and the maternal mortality rate. The top three causes of maternal deaths were obstetric hemorrhage, hypertensive disorders of pregnancy and cerebral hemorrhage. There were 39 maternal death cases in Shannan city during 2010-2017, and direct obstetric death cases accounted for 76.92% (30/39), of which 38.46% cases died of obstetric hemorrhage. The results of the WHO twelve-grade classification standard on maternal deaths showed that the cause regarding knowledge and skills of individuals and families accounted for 74.36%, the cause concerning knowledge and skills of medical institutions 61.53%, the cause about personal attitude 30.77%, and the cause with respect to management of health care institutions 15.39%. Conclusions Enhancing health education and management of maternal health care, guiding people to advocate science and awakening self-health awareness, intensifying the training of medical professional teams and special training targeted on diseases which can cause maternal death, and further improving the abilities in treatment, screening and timely referral of local high-risk pregnant and lying-in women are the key measures to reduce maternal mortality in Shannan city.
