Abstract:Objective To understand the endemic situation and risk factors of schistosomiasis in marshland regions, so as to provide evidences for appropriate control strategy and effective measure. Methods One hundred and twenty-one schistosomiasis endemic villages from 22 townships were elected as surveillance sites with a stratified and cluster sampling method in 5 marsh type counties around Dongting Lake and the middle of Yangtze River in China. The epidemiological data of the geographical environment, infections of schistosomiasis in residents and domestic animals, Livestock feeding way, Oncomelania snail status and relevant risk factors were surveyed and analyzed for the changes and causes of endemic situation from 2012 to 2013. Results A total of 119 708 local residents aged 6-65, 19 828 bovines (a main source of infection) and 10 646.22 hm2 snail habitat environments had been surveyed in the 5 counties from 2012 to 2013. The results showed that the average infection rate of schistosomiasis in the residents and bovines were respectively 0.85% and 0.48% and no infected snails were detected in the surveillance areas by the end of 2013, which decreased respectively from 2012 levels by 32.98%, 64.55% and 100.0%. The endemic situation was obviously lower than past years (P<0.05), but still higher than in other areas of the country in the same year. The main influence factors on schistosomiasis remained domestic animals opened on areas with snails (Multiple R=0.64, P<0.05) and occupational groups with higher frequency of contacting with schistosome cercariae water (RR=1.75). Conclusion The marshland regions is still a key area need to be controlled for schistosomiasis from present to the near future. The synthetical control measure with focus on domestic animals source of infection as well as the examination and treatment of the patients and snails control should be further strengthened.
