Abstract:Objective To investigate the health status and multiple chronic conditions of the long-lived elderly in Zhongxiang City, Hubei Province, and to provide references for prevention and treatment of multiple chronic conditions in the long-lived population. Methods With a random sampling method, the uniformly trained staff conducted physical health examinations,including physical check-up and laboratory examination, and a household questionnaire survey with Chinese Standard for the Healthy Elderly (2013) among the long-lived elderly ≥ 90 years in Zhongxiang, Hubei Province. χ2 test and Fisher's precision probability test were used to compare gender differences and distribution characteristics of multiple chronic conditions. Results A total of 323 long-lived elderly subjects were enrolled in this study, with the ages of 90-113[94, (91, 100)]years. Males and females accounted for 33.44% and 66.56%, respectively. The prevalence rate of chronic diseases was 98.45%, and the top three diseases were hypertension (80.50%), anemia (40.25%) and hearing loss (38.70%). The proportions of bone fracture(χ2=7.097, P=0.008) and cerebrovascular disease(χ2=4.269,P=0.039) were higher in females than in males, but the scores of activities of daily living(Z=-3.136,P=0.002), mini-mental state examination(Z=-4.709,P<0.001) and mini-nutritional assessment (Z=-2.403, P=0.016) were lower in females than in males. Each subject suffered from 0-8 [3(2,4)] kinds of chronic diseases. 87.31% of the long-lived elderly suffered from ≥ 2 kinds of chronic diseases, and most subjects suffered from 2-4 kinds of chronic diseases. Hypertension existed in the pattern of multiple chronic conditions. The common combination of binary diseases included hypertension + anemia(25.00%), hypertension + dyslipidemia(14.37%)and hypertension + hearing loss (14.37%). The common combination of ternary diseases included hypertension + vision loss + hearing loss(12.82%), hypertension + chronic kidney disease + anemia(10.26%)and hypertension + dyslipidemia + hearing loss(6.41%). Chronic disease co-morbidity was associated with age, living environment, dysfunction, cognitive dysfunction and depression. The proportions of subjects with dysfunction, cognitive dysfunction, depression and malnutrition increased with the accumulation of number of chronic diseases. Conclusion The longevous in Zhongxiang, Hubei Province are generally in good health, but it is common to co-exist with several kinds of chronic diseases. Geriatric syndrome is prominent with the accumulation of co-morbidities. It is necessary to strengthen chronic disease prevention and treatment according to the characteristics of multiple chronic conditions in the long-lived elderly so as to further improve their health status.
