Abstract:Objective To evaluate the blocking effect of combined immunization of high-titer hepatitis B immune globulin (HBIG) and hepatitis B (HepB) vaccine on HBV maternal-neonatal transmission. Methods Eight hundred and thirty-two infants born by hepatitis B surface antigen (HBsAg) positive mothers in hospitals of Danyang City in 2011-2015 were inoculated with 100 IU HBIG and 10 μg recombinant HepB vaccine originated from Saccharomyces cerevisiae within 12 h after delivery, and received full course of routine vaccination at 0-, 1- and 6-month age. Blood samples were collected from the children at the age of > 12 months for quantitative analysis of HBsAg and hepatitis B virus surface antibody (anti-HBs). The information concerning birth hospital and delivery mode was surveyed, and their correlations with the blocking effect on mother-to-infant transmission of HBV were analyzed. Results Among infants delivered by 832 HBsAg-positive mothers, 34 were positive for HBsAg, the efficacy of active immunization was 95.91%, and the positive rate of anti-HBs was 95.31% (793/832). The blocking rate of the infants born to HBeAg-positive mothers was lower than that of the infants born to HBeAg-negative ones (90.95% vs. 97.47%, χ2=16.409, P<0.0001), and their positive rate of anti-HBs was also significanlty lower (92.46% vs. 96.21%, χ2=4.756, P=0.029). No correlations were found between the blocking effect and delivery hospital, delivery mode as well as monitoring age. Conclusions Combination of HBIG with HepB is highly effective in blocking maternal-neonatal vertical transmission of hepatitis B virus, and it is not affected by the delivery hospital, delivery mode and monitoring age.
