Trends in incidence and death of upper gastrointestinal cancer among residents in Neixiang County, Nanyang City, 2014-2018

Abstract:Objective To analyze the changing trends in incidence and mortality of upper gastrointestinal cancer (including esophageal cancer and gastric cancer) among residents in Neixiang County, Nanyang City from 2014 to 2018. Methods The incidence and mortality data of upper gastrointestinal cancer in residents from Neixiang County during 2014-2018 were collected. Joinpoint 4.7.0 software was used to calculate the annual percentage change (APC) and 95% confidence interval (CI), crude morbidity, crude mortality, incidence rate by Chinese standard population, incidence rate by world standard population, mortality rate by Chinese standard population, and mortality rate by world standard population. We collected part of screening cases of upper gastrointestinal cancer and cases seeking medical treatment due to symptoms, and then observed the changes in their survival rates. χ2 test was used for comparison between groups. Results The average incidence rate of upper gastrointestinal cancer in residents from Neixiang County in 2014-2018 was 118.91/100,000, and the average mortality rate was 97.79/100,000. The incidence and mortality rates were higher in males than in females (both P<0.01). The incidence and mortality rates of upper gastrointestinal cancer both showed a downward tread. The APCs of crude morbidity, incidence rate by Chinese standard population, and incidence rate by world standard population were -6.63%, -6.72% and -7.13%, respectively (all P<0.01). The APCs of crude mortality rate, mortality rate by Chinese standard population, and mortality rate by world standard population were -6.22%, -6.91% and -6.81%, respectively (all P<0.01). The incidence rate increased rapidly from the age of 55 years, and reached the peak in 65-74 years old. The difference between mortality and incidence rates was that the peak age of 70-84 years was late and lasted for a long time. The 5-year survival rate of the screened cases was 84.27%, and the 5-year survival rate of the cases seeking medical treatment due to symptoms was 34.83%, showing a statistically significant difference (χ2=45.153, P<0.01). Conclusion The incidence and mortality of upper gastrointestinal cancer in residents from Neixiang County, Nanyang City during 2014-2018 showed a decreasing trend. Upper gastrointestinal cancer screening can improve the patients' survival rate.
