Current status and influencing factors of electronic cigarette and cigarette use among adolescents in Haikou City

Abstract:Objective To survey the current situation of exposure to cigarette and electronic cigarette (e-cigarette) among adolescents in Haikou City, and to analyze the correlations among different tobacco use behaviors so as to provide a reference basis for formulating tobacco control strategies and carrying out tobacco control education for local adolescents. Methods From May to July 2019, a multi-stage stratified cluster random sampling method and the core questionnaire on Global Youth Tobacco Survey (GYTS) were used to investigate students from middle schools, high schools and vocational high schools in Haikou City, Hainan Province. Multivariate logistic regression was used to analyze the factors influencing e-cigarette use and propensity to use. Results A total of 3,343 respondents were surveyed, and 3,273 valid questionnaires were collected, with a response rate of 97.91%. The average age of the surveyed adolescents was (14.60±1.78) years old, among which 1,637 (50.02%) were males and 1,636 (49.98%) females. 86.22% of teenagers had ever heard of e-cigarettes, and 13.29% had ever used e-cigarettes. The current use rate of e-cigarettes was 2.96%, and the current cigarette smoking rate 3.27%. 22.49% of the adolescents were exposed to e-cigarette advertisement, and the risks of e-cigarette use by friends' giving (OR=1.65, 95%CI:1.33-2.03) and e-cigarette use in the next 12 months (OR=1.68, 95%CI:1.31-2.15) significantly increased among adolescents with this exposure experience. After adjusting for gender, age and amount of pocket money, attempted, current and regular cigarette use could significantly increase the risks of e-cigarette use and propensity to use (P<0.05). Conclusion The use rate of e-cigarettes among teenagers in Haikou City in 2019 was slightly higher than the national average in the same period. Youth exposure to e-cigarette advertising can increase their tendency to use e-cigarettes; meanwhile, traditional cigarette use can also increase their e-cigarette consumption and propensity to use.
