Epidemic monitoring of schistosomiasis after interruption of transmission in Zhenjiang City, Jiangsu Province

Abstract:Objective To analyze the data about schistosomiasis epidemic situation in Zhenjiang City after interruption of transmission so as to provide a scientific basis for promoting the process of schistosomiasis elimination in the next stage. Methods We collected the annual schistosomiasis control data in Zhenjiang City from 2018 to 2021, and analyzed the infection status of people and livestock as well as the distribution of Oncomelania hupensis. Results There was no schistosomiasis emergency epidemic in Zhenjinag City during the period of2018-2021. A total of 340,259 person-time individuals were cumulatively examined, and the cumulative number of positive blood examinations was 2,182 person-time individuals. The positive rates of blood examinations in each year(2018-2021) were 0.80%, 0.79%, 0.58% and 0.38% respectively, showing a downward trend (χ2trend=164.034, P=0.001). The cumulative number of stool examination was 34,182 person-time individuals, and no positive case was detected.A total 13,323 herd-time livestock were tested for schistosomiasis, and nostool-positive resultswere found. From 2018 to 2021, the total investigated environmental area of the city was 55,172.67 hm2. The Oncomelania hupensis area in 2021 (817.08 hm2) increased by 29.17% compared with that in 2018 (632.57 hm2). Among the total area with Oncomelania hupensis, marshland accounted for more than 85%in each year.The total area of the niclosamide-controlled and environmental transformation was 12,365.64 hm2 and 560.75 hm2 respectively.A total of210,090 living Oncomelania hupensis were captured, the proportion of living Oncomelania hupensis frame in 2021 increased by 11.05% compared with that in 2018. The density of living Oncomelania hupensis was 1.16-1.35 snails/0.1m2. However, no Schistosoma japonicum infections were detected in Oncomelania hupensis by usingmicroscopic dissection. Conclusion The effect of schistosomiasis control measures focusing on the control of infectious sources in Zhenjiang City is remarkable, and the schistosomiasis epidemic after interruption of transmission has been maintained at a low level. However, the area with Oncomelania hupensis is still large, and the risk factors of transmission still exist. Measures to controlOncomelania hupensis should be further strengthened in future so as to promote the process of eliminating schistosomiasis.
