Changes in maternal mortality rate and principal component analysis of health care status in Lanzhou City, 2003-2018

Abstract:Objective To analyze the changes in maternal mortality rate and maternal health care status in Lanzhou City from 2003 to 2018, and to provide a basis for women's health care work in Lanzhou City. Methods The dynamic series and Mann-Kendall trend test were used to analyze the changing trends in maternal mortality and maternal health care status, and the principal component analysis was employed to evaluate the maternal health care status in Lanzhou City from 2003 to 2018. Results The maternal mortality rate in Lanzhou City during 2003-2018 showed a general downward trend, with an average growth rate of -3.46%. No statistically significant difference was found in the trend changes (Z=-0.855, P=0.392); moreover, there was a significant fluctuation during 2003-2018. The prenatal detection rate, early detection rate, postpartum visit rate, system management rate and hospital delivery rate in Lanzhou City from 2003 to 2018 showed an upward trend on the whole, with the average growth rates of 0.76%, 1.97%, 0.61%, 2.06% and 1.65%, respectively. The proportion of high-risk pregnant women showed a downward trend, with an annual average decline rate of 2.86%. The principal component analysis revealed that the level of maternal health care in Lanzhou City during 2003-2008 was lower than the average level during 2003-2018. The indicators of basic maternal health care services such as prenatal detection rate, early detection rate, postpartum visit rate, system management rate and hospital delivery rate from 2011 to 2013 were high. The proportion of high-risk pregnant women and the utilization rate of basic health care services from 2014 to 2018 were both declined as compared with those from 2011 to 2013. Conclusion Much progress had been made in maternal health care in Lanzhou City from 2003 to 2018, but the maternal mortality rates in 2016-2018 were higher than those of the national average level. It is still essential to continuously strengthen maternal health care services and improve their utilization.
