Abstract:Objective To investigate the ability of rehabilitation service of community family physicians and the situation of bodily function improvement, satisfaction for rehabilitation service and medical expenditure among the disabled receiving 1+1+1 contracted health services in Jinshan Industrial Zone, Shanghai. Methods Self-designed questionnaires were used to survey general information and types of rehabilitation service skills of 14 community family physicians and general demographic information, disability characteristics, situation of bodily function improvement, satisfaction for rehabilitation service and medical expenditure in 733 disabled people from August to October in 2016 and 2017. Results The average age of 14 community familyphysicians was in the range of 20-50 years old. They mastered many rehabilitation service skills and could provide personalized 1+1+1 contracted health services for the disabled. Among the 733 disabled people, the male-to-female ratio was 1.08:1. Most of the disabled were limb impairment (45.57%, 334/733). The IV-rating disabled were in the majority (46.66%, 342/733). As compared with theteam services by family physicians, the number ofthe disabled receivingdoor-to-door service and the number ofthe disabled withrehabilitation therapy by 1+1+1 contracted health services respectively increased 130 and 312. 186 (25.38%) disabled people’s bodily function was improved, and 47(6.41%) recovered.The satisfaction of the disabled with thecontracted services was improved, and the number of the disabled with satisfaction and high satisfaction increased from 585 (79.81%) before the contracted services to 714 (97.41%) after the contracted services. The medical expenditure of the disabled with the contracted services showed a downward trend, decreasing by 22.82% year-on-year. Conclusions 1+1+1 contracted health services of community family physicians can enhance the disabled’s satisfaction with rehabilitation service, improve their bodily function, and reduce their medical expenditure.
